[KI-LC] Preview of New GPA Signup Page w/ Consent Receipt

Thanks for the feedback Eve.
The policy link itself already changes based on the selection of the
workgroup. What is missing is that the name (so the "Kantara Initiative IPR
Policy Option") changes -- so it matches the link. This is on my todo list.
Also I will change the link (as well as the other links) to open a new
window/tab -- good point.
Company/Affiliation: During development we took the instructions out on
purpose. The idea was to reduce the amount of text one has to read in order
to signup and at the same time to visually improve the form. We have the
instructions on the current GPA signup and new users are supposed to fill
in their complete name if they sign up as an individual. But most seem to
ignore the instructions anyway and fill in things like "freelancer",
Some more info on the consent receipt is a great idea in my opinion. I
actually left out the download option for the consent receipt because it
would only confuse people without further info on what the file is and what
to do with it.
One more comment to the consent receipt. With this preview it is enabled
for all groups. But the final version will allow us to enable or disable
consent receipt on a group level -- in case we want it to roll it out for
only some groups at the beginning.
